Our policy is to accept and participate with all insurances, and to credential with all new miscellaneous insurances, treating them as a contracted insurance from a billing standpoint. If your insurance company is not listed below you may have a newer health insurance policy that we have not contracted with.

A Note About Referrals

It is extremely important to be aware of your insurance company's policy for referrals. A referral is a formal authorization from your insurance company, usually for the purpose of approving diagnostic or specialist studies or specialized services such as physical therapy. It is not the same thing as a doctor's order or prescription for treatment, because it comes from your insurance company and is requested by calling your primary care physician. The following insurances currently require an referral for our services:



  • Blue Care Network
  • Total Healthcare
  • Midwest Health Plan HMO
  • Health Choice (MHP subsidiary)
  • Tricare (unless secondary to another insurance)
  • Care Source
  • Oakland Health Plan


Upper Peninsula Health Plan requires a referral for all physicians not located in the Upper Peninsula of Michigan. EMG's are not a covered benefit under Macomb Health Plan. Referrals are submitted by your primary care physician. If you are unsure whether your insurance requires a referral (sometimes called pre-authorization) for the EMG/NCS test, please call the customer service number on your insurance card to inquire about referral policies.
